Coronary artery bypass grafting is usually performed on patients who are suffering from serious coronary artery disease. In coronary artery disease, a fatty material which is referred as 'plaque' gets accumulated inside the coronary arteries. The plaque can harden and narrow the lumen of the coronary arteries and reduce the flow of blood and oxygen to the heart, which can cause chest pain or discomfort called angina. This procedure improves the blood flow to the heart.
Different Types
There are different types of coronary artery bypass grafting which are recommended by the surgeon depending upon the patient’s complications in coronary arteries.
1. Traditional coronary artery bypasses grafting
Traditional bypass grafting is performed when at least one major artery is needed to be bypassed. During this procedure, the breast bone is cut for direct access of the heart. The heart-lung machine is connected to pump blood and provide uninterrupted blood circulation in the body. The heart is then stopped for some time so that the surgeon can perform the bypass surgery. After the surgery, the blood flow through the heart is restored. Usually, the heart starts beating again on its own or sometimes mild electric shocks are also given to restart the heart.
2. Off-pump coronary artery bypass grafting
Off-pump coronary artery bypass grafting is considered as the new method of performing CABG. This procedure is performed with the heart beating and without the use of the machine. This technique introduces a new complexity of attaching grafts to the heart while it is constantly functioning and filled with blood. Special devices are used at the time of performing the procedure as it can mechanically stabilize the relevant part of the heart so that the suturing can be performed on a relatively immobile platform. This is a highly specialized procedure.
3. Minimally invasive direct coronary artery bypass grafting (MIDCAB)
Minimally invasive bypass grafting is a new procedure. In this procedure, several small cuts are made on the left side of the chest between the ribs. The breast bone isn't opened to reach the heart. This type of surgery is mainly used to bypass blood vessels in the front of the heart. Minimally invasive CABG is a beating heart surgery, which means that stopping the heart is not necessary and a heart-lung machine is not required.
